A recent engagement: an account with 42 conversion actions and not one real recorded enquiry. The Google Ads tag on the site pointed at an ID that had never existed — someone years earlier had copied an internal account reference out of the browser's address bar and prefixed it with "AW-". Proved it against Google's own tag server, rebuilt the goals around actual enquiries, cleared nine dead actions, and confirmed a real conversion firing on the live site. Verified, not assumed.
